Otesse ships with 77 pre-built document templates spanning every compliance need for field service operations. Templates are organized into three top-level categories and seven subcategories, giving administrators a clear taxonomy for managing onboarding paperwork, policy acknowledgments, and operational certifications.

The total of 77 templates covers the core document needs for a service company operating across multiple US states. Each template is pre-configured with the correct signature requirements, renewal schedules, and blocking rules so that administrators do not need to build forms from scratch.

Employment Templates (52)

Employment templates handle everything from the first day of onboarding through separation. They break down into four subcategories:

Federal Tax (5 templates)

These are the federally mandated forms every US employer must collect:

Blocking templates prevent workers from being assigned to jobs until they are completed. The W-4 and W-9 renew annually on December 31, ensuring employees update their withholding elections each tax year. The I-9 uses signature-based renewal, expiring 36 months after the employee signs it.

State Tax (42 templates)

Every state that imposes an income tax has its own withholding form. Otesse includes templates for all 42 applicable states (the nine states without income tax — AK, FL, NV, NH, SD, TN, TX, WA, WY — are excluded).

All 42 state forms share the same configuration:

  • Blocking: Yes (all activities)
  • Signature: Required
  • Renewal: Calendar (December 31)
  • Grace Period: 30 days
  • Source URL: Links to the official state department of revenue form

In the UI, state tax templates are automatically grouped into a single expandable row labeled "State Tax Withholding (42 states)" rather than displaying 42 individual rows. Clicking the group opens a two-column drawer where administrators can browse states on the left and view individual template details on the right.

Benefits (5 templates)

Benefits documents cover health insurance enrollment, retirement plans, life insurance beneficiaries, PTO requests, and workers' compensation notices. None of these block job assignment — they are important for employee welfare but not legally required before a worker can start.

Separation (5 templates)

Separation documents are used during offboarding: voluntary resignation forms, exit interviews, final paycheck authorization, return of company property acknowledgments, and COBRA benefits notices. These are created on demand when an employee departs.

Policy Templates (10)

Company policy templates establish behavioral expectations. All 10 templates require a signature and renew annually on January 1, ensuring employees re-acknowledge policies at the start of each calendar year.

Examples include the Employee Handbook Acknowledgment, Anti-Harassment Policy, Code of Conduct Agreement, Non-Disclosure Agreement, and Data Privacy Policy. The NDA is an exception — it uses signature-based renewal (12 months from signing) rather than the calendar renewal used by other policy documents.

Operational Templates (10)

Operational templates cover equipment use, vehicle agreements, safety training, and background checks. Two subcategories exist:

  • Safety (5): Safety Training Certification, Emergency Contact Form, Incident Report, HAZMAT Certification, Background Check Consent

Vehicle-related templates use a special blocking scope — they only prevent assignment to jobs that require driving, rather than blocking all job types. This allows a worker with an expired Vehicle Insurance Proof to still be assigned non-driving jobs.

Template Grouping Logic

State-specific templates are grouped in the UI to prevent visual overload. The grouping algorithm works as follows:

  1. Templates with both a state value and a subcategory value are candidates for grouping
  2. Templates are grouped by the key {category}:{subcategory}
  3. Groups with only one template are demoted to singles
  4. The group display name is derived from the first template's name by stripping the state suffix after the last dash
  5. Groups render before singles in the list

This means the 42 state tax withholding forms collapse into one row, while unique templates like the W-4 and I-9 display individually.
